In compliance with the Affordable Care Act (ACA) reporting requirements, Form 1095-B, Health Coverage, is available upon request for covered individuals. This form provides information about the months of the year you and your covered dependents had minimum essential health coverage.

Upon receiving your request, we will provide the form within 30 days. Please ensure you provide your full name, policy number or member ID, and current mailing address in your request.

Letters have been mailed to each member regarding the new Online Dues Portal.  In the letter it explains how to set-up your account, which needs to be done first, as well as a 24-hour period for the account to be processed before you are able to submit a dues payment.  A link to this portal will also be added to our SMART Local #112 website.   We ask that you please be patient as we implement this new dues paying option to our system.

It is the decision of the SMW Local #112 Trustee’s to change our HRA Third Party Administrators from BRI to The Harrison Group (HG). We feel that this change will be a benefit to our participating members and the manageability of these funds. Please be aware that the last day you will be able to use your BRI card will be Monday, March 31, 2025. After that date, you will no longer be able to use your BRI card to access your HRA monies.

In the next few days any participating members should be receiving their new HG cards in the mail. You will need to be sure to activate your card once you receive them. However, initially the only monies that will be on this card are monies that are received from contractor contributions received after April 1, 2025. The monies that were on your BRI card will be transferred to your new HG card, but they will not be available until on or around April 10, 2025. We need to give BRI time to process any run-out claims before final balances can be transferred.

There will also be a new website/phone app for HG so that you can track your HRA card usage; submit claims or submit receipts for random audits.  This information had been posted on the Health Benefits page of this website and includes how to access the new website/phone app.

We appreciate your patience as we make this transition from one carrier to the other.

If you currently have a loan through Empower that you are making payments on, then the address you mail your quarterly loan payments to has been changed to the following:
